PROOF OF VACCINATIONS

Bluegrass Kennels requires a current shot
record (with the rabies tag number on it) from
a licensed veterinarian.  Your pet must be up-
to-date on the following vaccinations:

For Dogs:
~ Rabies *
~ DHLPP or DA2PC
~ Bordetella

For Cats:
~ Feline Viral Rhinotracheitis Calicivirus
   PanLeukopenia (FVRCP)
~ Rabies *

* All vaccines are annual with the exception
of Rabies which can be a three year
vaccination.
